This tutorial was written by me on October 9, 2012.
I used PSPX2 version to write it but any version is good.
Any similarities is purely coincidental.
If you're doing this tutorial you can get around PSP.

PTU Night Life

Artist: Gio - Little Black Skirt HERE
Mask: DBV Mask 110 HERE
Scrapkit: SKDS_I Love Nightlife - Nightlife Collab in PSP Project
Plugins: xero - porcelain, filter factory A - Pool Shadow
Font: I used Lemondrop and Carlotta

File - New - 600x600 Transparent Raster
Flood fill with white

Open ele14, resize by 90%, adjust - sharpen, c/p as a new layer
Add drop shadow 1 -1 50 5

Open ele07, c/p as a new layer, move to the right of the tag
Duplicate, image - mirror, add drop shadow 1 -1 50 5

Open ele01, c/p as a new layer, move down and to the left some
Duplicate, image - mirror
Drop shadow same as above
merge down - duplicate - then move up some - merge down
Duplicate layer, change blend mode to screen

Open your tube, c/p as a new layer, resize to fit tag
See mine for reference
Xero - Porcain (default), change blue channel to 0
Add drop shadow same as above

Open ele06, resize by 80%, adjust - sharpen, c/p as a new layer
Move down so its covering the tube's legs so they dont look cut off
Add drop shadow same as above

Open ele03, resize by 50%, then by 70% 
adjust - sharpen, c/p as a new layer
Move to the right of the tag and add drop shadow
Duplicate, image - mirror

Open ele05, resize by 50%, adjust - sharpen, c/p as a new layer
Move to the right of the tag and cover the end of the string, add drop shadow
Duplicate, image - mirror

Open your mask and minimize it in your workspace
Back to tag go to background layer
Open p03, resize to 600x600, c/p as a new layer
Layers - new mask layer - from image
Source window: find your mask
Create mask from source luminance
Merge Group

Close background layer and merge visible

With lemondrop font 48, anti-atlas smooth, stroke 1.0
foreground color: #3286ae
background color: #306a90
Change background to gradient: 
Foreground/Background gradient
angle 45, repeats 3, linear
Write Nightlife - convert to raster
Selections - all, float, defloat
Effects - plugins - filters unlimited - filter factory A - Pool Shadow
Default Settings
Add drop shadow 3 -3 70 10 black
Merge down

Add your credits and your name and you are done
